Online Directory Forms



Forms Overview


OIT Network IDentifiers (NetIDs) are used to provide authentication for many network-based services provided by the University of Tennessee. These include email accounts, wireless access, access to library services and lab computers, and many other systems. The NetID is associated with an entry in the online (white pages) directory, although this entry may be hidden for privacy reasons.

While students, faculty, and staff are added to the online directory (LDAP) and provided with NetIDs automatically through regular scheduled updates, the following forms are provided to handle special circumstances, such as adding a new employee who needs services before they arrive, providing access to a contractor or other person not paid through the University payroll system, or adding an email account for a campus organization.

Request for a Network IDentifier (NetID) for a Person


If a person is not currently in the online directory, they must be added before they can get a OIT email account or other computer and network services.

The Request for a Network Identifier is available in PDF. The PDF form can be filled out online using Interactive PDF, if you have a recent version of Adobe Acrobat Reader (Older versions of Acrobat Reader should be able to print the form.) Please print the form for any needed signatures and return according to the instructions on the form.

Request to add a Campus Organization to the Online Directory


Campus departments, organizations, or other units can receive an entry in the online directory. This entry can be used to set up a departmental email account or to redirect mail sent to an address of the form netid@utk.edu to an existing email address.

The Request to Add a Campus UNIT is available in PDF. The PDF form can be filled out online using Interactive PDF, if you have a recent version of Adobe Acrobat Reader (Older versions of Acrobat Reader should be able to print the form.) Please print the form for any needed signatures and return according to the instructions on the form.

Request to Sponsor a Person for OIT Services


Persons doing useful work for the University, but not paid through the University Payroll system (a contractor, for example) can be sponsored for access to OIT Services with a Dean, Director, or Department Head signature. This sponsorship must be renewed by submitting a new "Request to sponsor" yearly at the end of each fiscal-year. Notification that renewal is due will be sent to sponsored persons and their departments several weeks before services would be terminated. If the person does not already have a NetID, a Request for a Network Identifier should first be submitted.

Information for Retirees


Persons who have retired from the University with 10 years or more of service or faculty who have been granted Emeritus status by the University are eligible for continued use of their email account and NetID. Please visit the UT Benefits and Retirement page at http://www.utk.edu/benefit/ and fill out the "Continuation of Email and ERA Accounts" form.
