The following forms are provided to handle special circumstances, such as adding a new employee who needs services before they arrive, providing access to a contractor or other person not paid through the University payroll system, or adding an email account for a campus organization.

Please know that your request is important to us and it will be processed in the order that it was received. However, our response time at the start of the new semester will be slower than normal.

The owner of the account may contact the OIT HelpDesk for assistance. If the owner is not available, a memo may be sent on letterhead from the dean, director, or department head requesting the Display Name be changed.

Network Security Port Exemption Request

Network (LANMAN) Port Security Exemption Request

By default, all wired network ports restrict access to a single device (defined as a unique MAC address, a.k.a. Ethernet address). If you have a situation that requires the use of multiple MAC addresses behind a single network port, use this form to request an exemption from this policy and describe the reason(s) for your request. Submit this form by email to nsrequest@utk.edu.
